In accordance with the Ohio Charitable Solicitation Act, Put-in-Bay Community Swim & Sail Program, whose address is P.O. Box 881, Put-in-Bay, OH 43456, is registered with the Ohio Attorney General Office to solicit contributions in Ohio.  A copy of the Registration and Annual Filing can be obtained from the Ohio Attorney General Office   A copy of our annual report is available on our website at www.pibswimsail.org.  You may request a copy of the annual report and IRS 990-EZ filing by sending an email to pibswimsail@gmail.com and we will email you an electronic copy.
